With the Activate CareHub platform, St. Joseph Health, Queen of the Valley Medical Center's CARE Network has achieved:

- A decrease in both hospitalizations and emergency room utilization by about 64 percent

- 21 percent lower 30-day readmission rate for the CARE Network patients than the hospital's other patients
